Medical form D4 ....completed by a nurse

Your own GP does, that' why I use them,DVLA frequently ask for the records from the GP of the applicant especially if there is some medical history/problem.

This is what very often causes delays.

The answers you give to the questions are based on the information given to you by your GP (and maybe other medical professionals). If you answer the qustions on the form honestly then problems will be picked up. I agree though that if you do have any condition that requires DVLA access to your records using your GP for the C1 medical might speed things up.

The whole business of "Age Related" Medical checks for OLDER drivers. Is one thing that makes me ANGRY. IF it is such a "SAFETY" issue. Then there should be NO CHARGE whatsoever!. My view on ALL these type of things is "if Govt; demands it, Govt; should PAY for it!".

The Government (we taxpayers) does pay for the treadmill test - in my case a doctor and 3 nurses in a private room at local hospital

Must cost a small fortune

Fair comment but can you remember the last time you actually heard of a member of the public making an arrest? I vaguely remember one donkey's years ago and I think it fell because it hadn't been done properly but I think Prince Charles was at primary school then.

I appreciate that it's a fair while since I took notice but it was a regular occurrence when I was in a position to note it and that was 2001.
No doubt it continues today.

There seems to be an assumption that a GP knows all about everything to do with health and is an expert in dealing with driver medical issues. The trained nurse practitioner probably has more knowledge than the GP in this area and when you mentioned a condition she didnt know about said straight away that she needed to obtain information because of her lack of expertise. I would trust the nurse practitioner people who think they know everything are dangerous!

In heath these days there are mant people who are not doctors but have as much if not more knowledge in some areas to thinl it needs to be a doctor everytime is a bit old fashioned

I don't think anybody has a problem with a Nurse Practicioner/Prescriber actually doing the medical as long as they have access to a GP, which is always the case in a GP's surgery, it is the fact that at the moment it seems that they are not allowed to sign the D4 document.

My wife finished her nursing career as one of the early Nurse Prescribers 12 years ago.

I am booked in at my GP's surgery in a few weeks for my annual health check, this year for the first time the whole procedure can be carried out by a Nurse Practioner, blood samples etc have always been done by the Nurse, I would trust them more in taking blood than a GP, they have more practice

I don't know whether this is a recent introduction or that I have previously never been made aware of it.

I would be very wary of what happened. If the nurse had filled in the form and then had the doctor sign it I would have been content. If you had sent in the form innocently completely unaware that the signatory was not a doctor I would have been content. However, you established that she was not a doctor and so you could be said to be a party to a misrepresentation. That said the nurse signing as a doctor should, in my view, not have happened. I would clear it with DVLA.

Our surgery has a nurse practitioner who is able to diagnose certain thing and otherwise refer to the GP. Likewise she is able to sign prescriptions for a limited range of drugs otherwise she has to get a GP signature. There are good reason for this the main one being a different level of training, expertise, and responsibility.

If the department thought too many people with health problems were having accidents they would be down on it like a ton of bricks. Apart from the rare heart attack headline, I've never heard of any. Has anyone else?
