Problem with door hinge on a Autocruise Stardream

The lower door hinge on the habitation door of our Stardream has worked loose and it looks like it could be a nightmare to fix. The inner fixing nut is behind the door panel and inaccessible without removing the whole panel.. The external bolt is behind a cover which is proving impossible to remove without knowledge of how it is fixed. I'm hoping to find a way to remove the latter and then tightening the fixing bolt although how I can do that without holding the fixing nut I'm not sure. Have any of you had a similar problem with these hinges on a Stardream or similar model. Any advice gratefully received.

I don't know the model but, Is the bolt accessible to a drill?
Could you drill a small hole down the bolt centre and then, with a small bolt extractor, (left hand thread, use the bolt extractor to prevent the bolt turning as you tighten up the bolt.

Thanks for your reply. Part of the problem is that I can't figure out how to remove the decorative cover on the outside of the hinge. The fixings inside the door are hidden behind the door panel and also inaccessible. It's a nightmare!! All well and good during the original build but not easy to resolve now.
