French Vets for worming/pet passport completion prior to returning to the UK

There used to be a useful map/ list kindly put together by a Funster of the Vets for completing the PP prior to returning to the UK, but for the life of me I can't now find it - can anyone point me in the right direction please? I'm searching on my phone & the internet keeps dropping out so any help would be appreciated. Thanks

We went to the vets at Buchy on our way back to Calais from Normandy in March. They are superb only charged us 8 euros for worming tablet and sign off.

There’s a vet a Boug Achard, same practice as Mailleraye but I think they have better times, not as cheap as Buchy
