Where can you get the European Accident report form with NCR ( no carbon reqd)

Some time ago, I think perhaps from an Ins Co I was sent a set of these forms in different languages.
In the last year we've had a torrid time with Ins co's, luckily only needing the English version, but does anyone know where to get them from?.
I say NCR as if you download the form and print two copies you are asking for errors betwen the two versions, Ok so I know you can take a photo of each completed form but in the heat of an incident you need to keep things simple
So any idea where from please?

You can do it all electronicly with an app called "e-accident".

Also you will find that nearly all foreign drivers carry the form.

Yes but both drivers need one as I am signing yours along with you & you are signing mine as well as me.

Last time I used one their was two pages one me and one for the other party.
