I had a Safefill bottle that starting hissing, even with the handwheel off. A component of the valve itself had slightly unscrewed itself and needed nipping up with a special tool.

Could you smell gas in the cupboard when it was leaking?

It wasn't installed into a van. It was being used to power hobs on a scout camp at the time. There was the slightest smell of gas, it caused bubbles when connection tested and could be heard if you put your ear right next to it. Many people wouldn't have noticed, although installed into a van, the smell would have undoubtedly been stronger.
