Black ink?
Yes ,I'd missed that myself but the main problem is the 2019 -2020 middle one where the date at (2) is wrongly entered it has to be 21 days after the actual date administered.
Vet unique number missing on middle stamp?
they don't tend to worry about that as long as signed ,stamped ,dated & sealed

Only if it is the first recorded vaccination. See below:

Vet transferred all details hand written and stamped and signed. Sticker remains in uk passport but to point out the previous rabies vaccination from 2017 didn't have a sticker was hand written in the uk passport and have travelled fine with that several times.
Hi can i ask which vets in Lagos you used as there are a couple of vets practices there

Interestingly I then read p5 of the same booklet, which confirms that no permanent residential address is needed for an owner to be issued with a pet passport
If this is still the same since Brexit it could go a long way to answer the question a lot of us have been asking:

Out of interest I have tracked down the latest edition of the APHAā€™s ā€˜How to complete pet passportsā€™ (Aug 2020). Link below to full document:

A couple of points of interest (and this first one is the same as in the May 2018 version, I just hadnā€™t noticed it before):

805E21A4-3C9E-494D-AD04-8DB23C4619D4.png

The last paragraph is the interesting bit. If UK vets were able to issue pet passports to animals from Third Countries accompanied by an AHC, then there is no reason for EU vets not to do the same for us.

And the second interesting point (and this is a definite change from the May 2018 edition):

23913B6D-C130-4851-84F7-5C9471BE48B1.png

Read that last line ...... and then read it again! The bloody thing can now be completed in any colour ink. The reason I had to get my first one replaced was because the vet had completed it in black ink instead of blue - which was a cardinal sin at the time! Now it is apparently irrelevant.

We're of a similar mind, when we lived in Norfolk I worked at a vets, the vet owner told me that after the dog's initial inoculations & then the booster a year later, he didn't think there was much benefit having ongoing annual boosters. This was not the official story given to clients! šŸ¤£

We have had probably in excess of 30 dogs in our time together ( usually multiple dogs) some rescued some homebrew and some bought as puppies. I have only ever had their first set of jabs and annual boosters til 3yrs old and no more. I have never had a problem. Our dogs do not go into kennels but are with us all the time. We have had males castrated if they were a problem but never the bitches spayed. Again never a problem.

so it looks like MOST EU countries are on the 1 year booster but it does not appear at all clear even searching the various country information pages on the tinterweb which ones are more than 1 year.

Just heard from our friends (previously referred to) who are in the Algarve and have just got their dog a portuguese pet passport. They say that the Vets can give either a 1 year or a 3 year vaccination, they elected for the 3 year one. The cost was 15E for the passport and 10E for the rabies jab, they also paid 3E for a worming tablet at the same time. They suggest when booking for a rabies jab, ask if it's for 3 years, if not, try another Vet. They used the Municipal Vet at Albufeira.

Hope this helps! (y)

The difficulty in finding this sort of information was what prompted me to start this thread.

There is an interesting snippet of information in bellabee post #5 which I have translated from the German below:

So the 3-year vaccines will be available in various countries however you will need to check with individual vets that they are using them and, in some cases eg Spain, that regional laws actually permit their use.

they are lying to you.
It may also have something to do with the fact that dogs have a shorter lifespan and thus 'age' quicker relative to us, hence their immune system won't be as strong in later years and the need for top-up vacs more frequent.

There must be some truth in the length of time the vaccine works, because dogs who have had it over a year often fail the titre test. Maybe the dose you can give dogs is small, because they are small.
