I have no knowledge of your van but lots of oil cooler's (heat exchanger) are water cooled, Not one of my favorite ways of cooling oil, good in cold conditions for warming it up though.

Is it the one that is trapped(sandwiched) between the oil body & the oil filter & with 2 hoses attached to it ? If so yes it is a one piece unit & has a seal on the engine side as the seal on other side is provided by the filter. Nothing in it to leak unless it becomes perforated. The usual fault, that I had, was one of the 2 water pipes gets eaten away & becomes difficult to get a decent clamping with the jubilee clip. I just silver soldered alonger piece of pipe into the original.

Yes that's it or similar,i took it out for a spin as had been laid up for 2 months, then i noticed water under the engine bay did a bit of investigating and found it to be leaking,hopefully an o'ring then its due for a mot so will get them to look at it,Ive been googling it for days and not a thing of how to repair one and you found it straight away.(y)

That's a newer type to mine which is fitted where your green arrow for the filter is & then filter fits after.

@matt34
It might be worth asking on the fiat forum.
There are some knowledgeable guys to offer advice, although you might have your answer from Gus
