I would ( and do ) love to help folk out.. But to come on a forum with absolutely no preamble and without at least asking the forum owner to pick peoples brains.. not nice.. But I see Jim has dealt with the person in the appropriate manner

The OP was aged 18 and presumably a student. I'd be happy to fill in the survey, they just needed a bit of guidance given the target audience here.

Automated forms are one of the riskiest things there are to click on links for, and anyone can claim to be 18 and or a student. I'd recommend against it, for what it's worth.
